Dr. Jaclyn Tolentino, is a Board-Certified Family Physician with a collaborative, approach to medicine. She holds a subspecialty certification in Hormone Optimization, has received extensive training through the Institute for Functional Medicine, as well as a special interest in oncology nutrition. Dr. Tolentino is deeply committed to creating opportunities for her patients to make empowered and informed decisions about their own health, working together to explore personalized methods of treatment for sustainable, long-term wellness.
